Abstract
Rehabilitation of missing posterior teeth in the maxilla has many clinical challenges due to the limited bone height and poor bone quality associated with sinus pneumatization and ridge atrophy. Transalveolar sinus floor elevation using an osteotome-mediated technique has been known to be less invasive compared to the lateral approach; however, the conventional method has clinical limitations when it is used in cases with remaining bone height less than 5 mm and anatomic variations, and it is associated with patient discomfort caused by tapping the osteotome. In the present case report, two alternative devices (Hydro membrane lift system and Hatchreamer) were introduced for a modification of the conventional osteotome technique. By using the specially designed drills, reamers, and hydraulic lift caps, it became possible to perform less invasive sinus floor elevation with reduced risk of membrane perforation. Adequate vertical sinus augmentation with a dome-shaped radiopacity around the implant apex was achieved. Also, patients were satisfied with the methods as they experienced less complications during the surgery and postoperatively. Based on the results in this report, alternative devices such as Hydro membrane lift system and Hatchreamer were effective and noninvasive methods that can be used for the transalveolar approach.
